My fic always tends to be heavy on dialogue and low on description, so this time I pushed that to the extreme. I think this piece is best thought of as the script for a radio drama - and it's up to the actors (or the listener/reader) to decide what tone the characters use and what might be going on that they're not mentioning. Mycroft's reaction to John stripping and masturbating, in particular, is left entirely up to the reader.
